From the album Mechanical Animals, "Rock Is Dead" by Marilyn Manson climbed from #38 to #37 on the April 24, 1999 Modern Rock Tracks chart. The song would hit #30, and reach a similar peak of #28 on the Mainstream Rock Tracks chart. NSFW lyric warning!
Also from the same album: The uncharted "Coma White", whose video features Manson's recreation of the Kennedy assassination. Controversial at the time, due to Columbine and the death of JFK Jr. that same year.

Foo Fighters' cover of Gerry Rafferty's "Baker Street" was up to #36 on the Mainstream Rock Tracks chart for this week in 1998. Originally on the "My Hero" single release in the United Kingdom, the remake climbed to #34 two weeks later.

The Johnny Cash remake is the version that's the most famous, but Nine Inch Nails' original recording of "Hurt" from The Downward Spiral hit the Hot 100 Airplay chart back in 1995. This week that year, it climbed to #62 on its way to #54. NSFW lyrics warning!
Runners-up: Green Day - "She", The Magnificent Bastards - "Mockingbird Girl"

"Quiet" version (the version I remember being played on the radio back in 1995):
