I Can't Sleep Baby (If I) - R. Kelly This one he did mention the subtitle once (either the week it debuted or soon after), but never again after that.

During its initial run in 1977-78, I don't think Casey ever said "The Final Acclaim" after "You're In My Heart." However, he did mention it on his AC shows in the 2000s.

He never said the full title of Love Is In Control (Finger on the Trigger).

Did Casey ever say the full title of Culture Club's Time (clock of the heart)? Somebody must know.

I don't remember Casey ever saying "Disco Round" following "I Love The Nightlife," although he may have on the AC version of AT20, or AT10.

I don't recall Casey mentioning either subtitle of "I Love The Nightlife" or "Time", not during the initial AT40 run. Good mention, johnnywest, on the AT20/AT10 shows - if any song with a subtitle was played in the show as an extra or #1 hit (this week in 19XX), I think the subtitle was mentioned more times than not.

This one always stands out to me whenever I hear Casey say it: he mostly referred to Glass Tiger's "Don't Forget Me (When I'm Gone)" as just "Don't Forget Me".

How about Lionel Richie's "All Night Long (All Night)"? I know the subtitle was added officially for publishing reasons, but to me, it ranks as possibly the most superfluous.

How about Mike + the Mechanics' "Silent Running (on Dangerous Ground)"?

When the Rolling Stones were in the Top 40 during 1974 with Doo Doo Doo Doo Doo (Heartbreaker)--Casey mostly only called the song Heartbreaker. Although I don't blame him as I only called the Otis Redding song Fa Fa Fa Fa Fa (Sad Song); by it's subtitle-- and I only announced Um, Um, Um , Um, Um, Um by Major Lance the Um Song (I wonder how DJ's back in the '60's handled those two songs).

Here's one that for some reason only has a subtitle on American Top 40 cue sheets and isn't heard anywhere else except in the lyrics, not even its 45 or album has, "I'll Play for You" with (Hear the Band) by Seals & Crofts!

What about "The Breakup Song (They Don't Write 'Em)" by The Greg Khin Band?

In a reverse situation, a lot of people seem to forget that a certain Eiffel 65 song from late 1999 had a subtitle...but Casey mentioned "Blue (Da Ba Dee)" in full on the Top 100 of 2000 show.
Among the populace however, so many people forgot the subtitle that some lyric sites still have misheard lyrics in place of the "da ba dee da ba dai" in the song.
